MDS Coordinator RN

About the Role:
The MDS Coordinator RN is responsible for coordinating the interdisciplinary team in the timely completion, accuracy, and transmittal of the Minimum Data Set and all corresponding assessments and care plans.
Key Responsibilities:
- Coordinate the interdisciplinary team to ensure timely completion of Minimum Data Set and assessments
- Ensure accuracy and transmittal of Minimum Data Set and care plans
- Collaborate with the Director of Subacute Nursing to achieve departmental goals
Requirements:
- Current California Registered Nurse license
- Two years recent SNF experience preferred
- Minimum of six months experience working as a MDS Coordinator preferred
- Computer skills required
- Current BLS (CPR) certificate required
