About the Role

We are seeking a skilled Physical Therapist to join our team at Fairview Health Services. As a Physical Therapist, you will play a critical role in helping patients reach their goals and live more fulfilling lives.

Key Responsibilities
  • Partner with patients and their care providers to develop and implement individualized plans of care
  • Conduct thorough evaluations and re-evaluations to determine treatment diagnoses and develop treatment plans
  • Implement and modify care plans based on patient response and reassessment
  • Communicate effectively with interdisciplinary teams to coordinate care and establish discharge plans
  • Provide skilled interventions and education to patients and families
Requirements
  • Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN
  • Current Basic Life Support (BLS) certification
  • Doctorate degree in Physical Therapy
  • 1+ year of Physical Therapy practice
  • 1+ year experience with pelvic health
