Job Summary

We are seeking a highly skilled Registered Dietitian to join our team at Elderwood. As a key member of our nutrition team, you will be responsible for providing expert nutrition care to our residents, developing and implementing individualized nutrition care plans, and conducting routine audits to ensure the highest quality of care.

Responsibilities
  • Conduct comprehensive nutrition assessments for adult and geriatric residents in a long-term care and sub-acute rehabilitation setting.
  • Develop, implement, and monitor individualized nutrition care plans based on current best practices.
  • Provide nutrition counseling and education to residents and their family members.
  • Maintain accurate and up-to-date resident nutrition information in our computer system.
  • Conduct routine audits and monitor performance improvement initiatives related to nutrition and food service.
  • Participate in resident care plan meetings, staff meetings, and other relevant meetings.
  • Stay current with federal and state regulations regarding nutritional services in long-term care settings.
Requirements
  • Bachelor of Science degree or higher in Nutrition/Dietetics from an accredited educational program.
  • Successful completion of an internship or equivalent clinical management practice qualifying for Professional Registration with the Commission on Dietetic Registration.
  • Active and ongoing Registration with the Commission on Dietetic Registration.
  • Active membership in The Academy of Nutrition and Dietetics preferred.
  • Certified Dietitian - Nutritionist (CDN) with The New York State Department of Education or Licensure in the state of employment (or to be obtained within one year from date of hire).
Preferred Qualifications
  • Long-term care experience.
  • Computer skills and knowledge in nutritional software programs.
