About the Role

PennState Health is seeking a highly skilled Pharmacy Technician to join our team in a Medication Reconciliation Specialist role. As a key member of our healthcare team, you will be responsible for working with newly admitted patients to obtain and document an accurate medication list, ensuring seamless care and minimizing medication errors.

Key Responsibilities
  • Collaborate with healthcare professionals to obtain and document accurate medication lists for newly admitted patients.
  • Enter medication information into electronic health records to ensure accurate and up-to-date patient information.
  • Work closely with physicians and other healthcare providers to ensure medication lists are accurate and up-to-date.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements
  • High School Diploma or equivalent required.
  • Current Pharmacy Technician certification or currently licensed to practice as a licensed practical nurse by state of employment or holds a multistate LPN license through the interstate Nurse Licensure Compact.
