Responsibilities

The successful candidate will be responsible for:

3. Collaborating with healthcare teams to ensure seamless care transitions and optimize patient outcomes;

Qualifications

To be considered for this role, applicants must:

• Hold a valid state driver's license and maintain reliable transportation;
• Be currently licensed as a Physical Therapist in the state of employment;
• Possess excellent communication and interpersonal skills;
• Demonstrate a strong commitment to delivering high-quality patient care and exceeding expectations;
• Meet all applicable state and federal regulations regarding healthcare provider requirements.
