Prokatchers is seeking a skilled Travel Nurse RN Case Manager to join our team. As a key member of our healthcare staff, you will be responsible for providing exceptional patient care and collaborating with multidisciplinary teams to optimize outcomes.
Key Responsibilities- Lead the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, and procedures.
- Discuss with physicians the appropriateness of resource utilization, consultations, treatment plan, estimated length of stay, and discharge plan.
- Focus on complex patients, frequent emergency department utilizers, chronic pain patients, substance abuse patients, and homeless patients.
- Collaborate with acute care case managers to ensure appropriateness of ongoing care.
- Coordinate the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services.
- Provide oversight and collect data required for regulatory and accreditation compliance.
- Manage frequent emergency department visitors by conducting a focus study review of the previous and current admissions.
- Bachelor's Degree in Nursing (BSN): Preferred
- Experience in a care management role: Preferred
- Need RN with acute Case Management experience recent within the last year in a hospital setting.
- Need discharge planning with knowledge of Utilization Review and Status.
- Registered Nurse (RN) licensure in the state of practice: Required
- Case management certification: Preferred
