Clinical Case Manager

As a Clinical Case Manager, you'll play a critical role in coordinating patient care plans, ensuring seamless transitions between care settings, and optimizing outcomes for our patients. Your expertise in assessing patient needs, developing care plans, and collaborating with interdisciplinary teams will be invaluable to our patients and families.
Key Responsibilities:
- Develop and implement comprehensive care plans, incorporating input from patients, family members, caregivers, and healthcare providers.
- Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing tailored care plans to address these needs.
- Collaborate with patients, family members, and caregivers to educate them on disease management, treatment options, and self-care strategies.
Requirements:
* Graduate of an accredited nursing program (BSN or MSN preferred)
* Current RN licensure in good standing
* 1+ years of experience in clinical care settings (home health, hospice, or acute care)
* Excellent communication and interpersonal skills, with ability to build strong relationships with patients, families, and colleagues
* Strong organizational and time management skills, with ability to prioritize multiple tasks and deadlines
* Reliable transportation to perform job duties, with valid driver's license and insurance
