Care Specialist

Centerstone is seeking a dedicated and compassionate Care Specialist to join our team. As a Care Specialist, you will play a vital role in providing person-centered assistance to clients in the development of life skills and personal goals. You will work closely with a multidisciplinary team to coordinate client care, connect clients with community resources, and provide crisis intervention/prevention to clients and their families.
Key Responsibilities- Coordinate client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client's clinical record, including treatment plans, progress notes, and reporting.
- Teach and train clients in activities of daily living, in group and individual settings, and provide services according to organizational guidelines and/or protocols.
- Connect clients with community support systems as determined to be appropriate, and maintain regular communications with schools, OFC's, and other family service providers.
- Provide telephone and on-site crisis intervention/prevention and follow-up to clients and their families.
- Associate's degree required. Two years of experience or two years of college coursework in a related field may be substituted for degree requirement.
- Two years of professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice, behavioral health, or related setting preferred.
