About the Role:

We are looking for a dedicated and experienced Registered Nurse to provide high-quality care to our patients in their homes. As a Per Diem Nurse, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education to patients and their families.

Responsibilities:
  • Assess and evaluate patient needs, developing and implementing individualized care plans
  • Provide skilled nursing care to patients in their homes
  • Document all changes in patient condition and update care plans and outcomes
  • Identify teaching needs and plan appropriate strategies for individuals and families
  • Establish and maintain professional working relationships with healthcare providers and other members of the interdisciplinary team
Requirements:
  • RN with Bachelor's degree in Nursing preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
