About the Role

We are seeking a compassionate and motivated Nurse Practitioner to join our team. As a primary care specialist, you will be responsible for managing the acute, subacute, and primary care needs of our patients. This will involve working closely with our team, nursing home staff, residents, and families to provide education on best practices and integrate various interventions, emphasizing non-pharmacological treatments whenever possible.

Responsibilities

• Round at assigned facilities to assess patient needs and develop care plans.
• Collaborate with the OnSite team to provide comprehensive care and support.
• Proactively follow up with patients to assess the efficacy of recommended interventions.
• Provide education on best practices and promote healthy behaviors.
• Master of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification.
• Active License in South Carolina.
• DEA registration.
• Excellent communication and documentation abilities.
• Strong patient assessment skills.
