Key Responsibilities:

  • Develop and maintain strong relationships with members to understand their needs and goals.
  • Collaborate with multidisciplinary teams to provide comprehensive care and support.
  • Conduct assessments and develop individualized plans to address member needs.
  • Provide education and support to members on mental health topics, self-care, and life skills.
  • Facilitate group sessions and workshops to promote social connections and skill-building.
  • Participate in case conferencing and consultation to ensure quality care and coordination.
  • Document all interactions and interventions in member records.

Requirements:

To be successful in this role, you should have a high school diploma or equivalent, a minimum of 2 years of experience in a related field, and a valid driver's license. A degree in psychology, social work, or a related field is preferred.
