**Job Summary**
We are seeking an experienced Nurse Practitioner to join our multidisciplinary healthcare team in Eugene, OR. As a key member of our primary care team, you will provide high-quality patient care, diagnostic assessments, treatment plans, and education to patients.
**Responsibilities:**
- Perform comprehensive physical exams and assessments.
- Order, interpret, and follow-up on diagnostic tests and laboratory results.
- Diagnose and manage acute and chronic conditions.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Prescribe medications, therapies, and treatments within the scope of practice.
- Provide patient education and counseling on disease prevention, wellness, and lifestyle changes.
**Qualifications:**
- Master's degree in Nursing from an accredited Nurse Practitioner program.
- Valid Nurse Practitioner license in the state of Oregon.
- Current certification as an Adult, Family, or Psychiatric Nurse Practitioner (or other appropriate certifications).
- Minimum of 2 years of experience as a Nurse Practitioner, preferably in a primary care or specialized setting.
- Current Basic Life Support (BLS) from the American Heart Association (AHA) or the American Red Cross.
- Covid Vaccinated.
- Recent PPD Test or Chest X-Ray.
- Clear criminal background check(s) & drug screening.
- Reliable transportation & smartphone.
