Home Health Nurse Position

We are seeking a compassionate and skilled Registered Nurse to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing personalized care to patients in their homes, helping them manage chronic conditions or recover from illness or injury.
As a Care Team Member, you will work closely with our multidisciplinary team of healthcare professionals to deliver high-quality patient-centered care. Your responsibilities will include conducting home visits, assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare providers to ensure seamless care coordination.
To succeed in this role, you will need strong communication and interpersonal skills, as well as the ability to think critically and make sound clinical judgments. You should be comfortable working in a fast-paced environment and have excellent time management skills.
**What We Offer**
In return for your hard work and dedication, we offer a competitive salary range of $70,500-$96,900 per year, based on experience and qualifications.
