Occupational Therapist PRN

We are seeking a skilled Occupational Therapist to deliver high-quality therapy interventions and treatment in a home health setting.
The ideal candidate will possess strong clinical expertise and excellent communication skills to collaborate with interdisciplinary team members and provide exceptional patient care.
Key Responsibilities:- Perform comprehensive assessments to evaluate patient mobility, coordination, strength, and functional abilities.
- Develop and implement individualized treatment plans to achieve patient goals and outcomes.
- Collaborate with healthcare team members to ensure seamless care coordination and effective communication.
- Document patient progress and outcomes in accordance with regulatory requirements and company policies.
- Participate in discharge planning and education to ensure a smooth transition for patients and their caregivers.
- Master's degree in Occupational Therapy from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapist in the state of practice.
- Valid CPR certification.
- Strong organizational and communication skills.
- Attention to detail and ability to manage multiple priorities.
- Proven relationship-building skills and ability to work effectively with diverse stakeholders.
