Job SummaryThe Patient Care Manager RN will be responsible for coordinating and supervising an interdisciplinary team of staff to provide comprehensive care to patients in their homes. This role requires strong leadership and communication skills, as well as the ability to work effectively with patients, families, and healthcare professionals.
Key Responsibilities- Coordinate and supervise the delivery of patient care services, ensuring that patients receive high-quality, individualized care.
- Develop and implement patient care plans, in collaboration with the patient's healthcare team.
- Conduct regular assessments of patient needs and make recommendations for changes to the care plan as needed.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Supervise and mentor staff members to ensure they have the necessary skills and knowledge to provide high-quality patient care.
- Participate in quality improvement initiatives to ensure that patient care services meet the highest standards.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
