Job Summary

Key Responsibilities
  • Case Management: Develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients and families.
  • Crisis Intervention: Provide immediate support and intervention to patients and families in crisis situations.
  • Biopsychosocial Assessments: Conduct thorough assessments of patients' physical, emotional, and social needs to identify areas for intervention and support.
  • Psychotherapy and Counseling: Provide individual, group, and family therapy to patients and families to address mental health concerns and promote overall well-being.
  • Referral and Linkage: Develop and maintain relationships with community resources and providers to ensure seamless referrals and linkages for patients and families.
Requirements
  • Education: Master's degree in Social Work from an accredited program.
  • Licensure: LCSW-C licensure from the Maryland Board of Social Work Examiners.
  • Experience: Minimum two years of post-Master's experience in clinical social work or a related field.
Preferred Qualifications
  • Knowledge and Skills: Demonstrated expertise in clinical social work, case management, and crisis intervention. Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
