Dignity Health Medical Foundation, a leading healthcare provider in California, is seeking an experienced care manager to support quality medication management and clinical problem-solving functions.

Key Responsibilities
  • Triage and Initial Screening: Provide exceptional customer service over the phone, utilizing strong communication skills to evaluate and respond to medication refill requests under the direction of licensed providers.
  • Treatment Planning and Patient Interaction: Develop treatment plans, observe patient interactions, and record interventions in the medical record, requesting prior authorization for medications as needed.
  • Clinical Support and Referrals: Refer patients or initiate charitable care special assistance applications, writing correspondence addressing medication-related concerns and administering injections.
  • Scheduling and Staff Management: Manage the psychiatrists' cancellation lists, schedule, and cancel patient appointments, attending and participating in staff and in-service training meetings.

This role requires a high level of independence, with the ability to use independent judgment to gather needed information and provide support to physicians. The ideal candidate will have a minimum of one year of experience working with psychiatric patients, equivalent experience as a Licensed Vocational Nurse (LVN), and possess a current unrestricted California licensure as a Psychiatric Technician or LVN.

Requirements
  • Bachelor's Degree in Nursing or related field
  • Current BLS through American Heart Association
  • CA Driver License
  • Able to administer injections and work effectively in a fast-paced environment

The estimated salary for this position is $60,000 - $80,000 per year, depending on experience. This position is represented by SEIU-UHW and offers competitive benefits and opportunities for professional growth.
