Key Responsibilities
- The Acute Care Clinical Case Coordinator evaluates the social, psychological, cultural, environmental, and financial factors affecting patients and their families.
- This role serves as an advocate for patients and families, upholding their rights and dignity while encouraging their involvement in care decisions.
- The Coordinator also manages the processes for acute, rehabilitative, and long-term discharges, ensuring a seamless age-appropriate continuum of care for the designated populations.
Qualifications
Essential Requirements:
- Minimum: Bachelor’s degree in Social Work or an Associate’s Degree in Nursing or Nursing Diploma.
- For Social Workers: A Medical Social Worker or Licensed Clinical Social Worker designation is preferred. A current license from the relevant board and basic life support certification is required.
- For Registered Nurses: A valid nursing license from the relevant state or compact license (to be obtained within 90 days of hire) and BLS certification; additional certifications such as ACLS or PALS may be required by the hiring department.
Preferred Qualifications:
- Master’s Degree in Social Work or Bachelor’s Degree in Nursing is desired.
- Two years of experience as a medical social worker or RN Case Manager is preferred.
- Certification in Case Management (CCM or ACM) is advantageous.
