Job Title: LPN - Full Time

Frontier Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a full-time capacity. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

The LPN will be responsible for providing professional nursing care to individuals within an assigned unit of the hospital and/or program. This includes assisting in the development of a plan of care, recording individual's activities, observations, and behavioral symptoms, and maintaining clinical case loads.

Key Responsibilities:
  • Assist in the development of a plan of care for patients.
  • Record individual's activities, observations, and behavioral symptoms per treatment plan and program procedure.
  • Maintain clinical case loads, including maintenance of appropriate contacts with referral sources, coordination of services with other professionals and agencies, and necessary record keeping.
  • Provide a variety of didactic therapeutic groups as per specific program requirements.
  • Assist physicians as needed or requested.
  • Evaluate and document individual's reactions to medications and treatments.
  • Administer prescribed medications per MD order.
  • Provide patient/family education regarding medication, treatment, diagnoses, and disease process.
  • Monitor/assess individual vital signs, observe and record medical needs, and responses to medical treatment.
  • Utilize personal protective equipment when performing invasive procedures, collecting urine, and transporting lab specimens.
