Job Overview:
The successful candidate will work with patients in the comfort of their own homes, utilizing their expertise to deliver personalized care and support. This rewarding role offers a unique opportunity to make a positive impact on patients' lives while enjoying a relaxing island setting.
About the Role:
This 16-week assignment requires a dedicated LPN to work independently, collaborating with healthcare teams to develop and implement individualized care plans. The ideal candidate will possess strong communication skills, be able to prioritize tasks effectively, and demonstrate empathy when interacting with patients and families.
Requirements:
To succeed in this role, candidates must meet the following requirements:
- HI license
- 2 years of experience as an LPN
- Current AHA BLS certification
- Preferably 1 year of home health experience
- Electronic documentation proficiency

- Communication of patient status changes to RN/MD
- Follow-up phone calls
- Patient health questionnaire/history/allergies
- Vital sign monitoring

Salary:
The estimated annual salary for this position is $95,000-$110,000, depending on experience and location.
