Key Responsibilities- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs
- Develop and implement individualized care plans, taking into account the unique needs and preferences of each resident
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
- Conduct regular assessments and evaluations to identify changes in resident condition and adjust care plans accordingly
- Document resident care and progress in a timely and accurate manner
- Current RN licensure in the state of Michigan
- Minimum of 1 year of experience in long-term care or a related field
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members
