Job Summary:
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at Valle Vista Post Acute. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents in a dynamic and fast-paced environment.
Responsibilities:
- Observe, report, document, and chart patients' conditions to ensure accurate and timely care.
- Administer medications in accordance with physician orders, prioritizing patient safety and well-being.
- Take and record vital signs, including temperature, respiration, and pulse, to monitor patients' conditions.
- Prepare treatment trays and equipment as needed, maintaining a clean and organized workspace.
- Review reports from previous shifts to determine changes in patient status and assign duties to Certified Nursing Assistants (CNAs) or other nursing personnel.
- Turn over accurate records and ensure oncoming shift nurses are updated, promoting seamless care transitions.
- Notify the Director of Nursing Services (DNS) and/or Administrator of changes in patient condition or other job-related issues.
Requirements:
- Current CA LVN license in good standing.
- 2+ years of experience in long-term care, preferably in a skilled nursing setting.
- CPR and First Aid certification.
- Outstanding communication skills and a positive, professional demeanor.
