**Eligibility Criteria:**
• Possess a valid Class A CDL
• No DWI or DUI convictions within the past five years
• No major preventable accidents within the past twelve months
• No reckless driving convictions in the last three years and never in a commercial vehicle
• Must have a 1 year DOT physical and pass a drug test
• Must be at least 23 years old

**Application Requirements:**
• Social Security Number (will be asked for during the interview)
• Home address history for the past 3 years
• Current drivers license number and drivers license history for the past 3 years
• Employment history for the last 3 years plus additional 7 years driving experience only
• History of traffic accidents, violations, and/or convictions from the last 3 years (including DUI or reckless driving convictions and license suspensions)
• Criminal history
• Military history (if applicable)
