Nursing Case Manager for Complex Care Coordination

Mass General Brigham is seeking an experienced Nursing Case Manager to facilitate discharge planning and coordinate post-acute care services for patients in various hospital departments. The successful candidate will possess a strong background in healthcare reimbursement, quality improvement, and transitional care.

Key Responsibilities:

  • Develop and implement discharge plans in collaboration with patients, families, healthcare teams, and insurers
  • Utilize risk screening criteria to identify high-risk patients requiring specialized care coordination
  • Communicate effectively with physicians, office staff, and healthcare providers to ensure appropriate level of care and compliance with regulations
  • Maintain accurate insurance reviews and communicate coverage limitations to patients and families
  • Serve as a resource for healthcare professionals, patients, and families on community resources, referrals, and post-acute care options

Requirements:

  • Graduate of an accredited nursing program; BSN preferred
  • Current licensure as a Registered Nurse in Massachusetts with at least 3 years' clinical experience
  • Experience in general acute care, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery
  • Knowledge of special needs and behaviors across the lifespan
  • Strong decision-making skills, autonomy, and problem-solving abilities

We offer a competitive salary range of $83,000 - $123,000 per year, depending on experience, as well as excellent benefits and opportunities for professional growth and development.
