Key Responsibilities- Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions.
- Collaborate with registered nurses and physicians to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
- Develop and maintain effective relationships with patients, families, and healthcare teams to ensure seamless care delivery.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Stay up-to-date with the latest clinical guidelines, policies, and procedures, applying this knowledge to improve patient outcomes.
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing (NLNAC).
- Full, active, current, and unrestricted licensure as a Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States, or District of Columbia.
- English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
- One year of experience as a Licensed Practical Nurse (LPN) in an ambulatory care setting.
- Knowledge of electronic medical records and experience with computerized documentation systems.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
