Physical Therapist-Home Health Care

As a KVNA Physical Therapist, you will work with patients to improve their movement and manage their pain, making a real-time difference in their recovery and quality of life.

Our physical therapists work with patients to develop personalized care plans, taking into account their unique needs and goals.

Duties and Responsibilities include:

  • Assessing and evaluating patients' therapeutic, rehabilitative, and functional status to develop a comprehensive plan of care
  • Assessing muscle strength, mobility, gait, and range of motion to identify potential for rehabilitation
  • Initiating physical therapy programs and instructing patients and their families/caregivers on the use and care of therapeutic appliances
  • Reporting to physicians on patients' responses to treatment and changes in condition
  • Identifying patients' and their families'/caregivers' needs for other home health services and referring as necessary
  • Communicating with physicians and other practitioners related to patients' current home health plans of care
  • Participating in discharge planning for patients
  • Supervising Physical Therapist Assistants according to policy and procedure and state regulations
  • Submitting thorough clinical notes and progress summaries based on achievement of goals
  • Participating in the agency's quality assessment and performance improvement program
Job Requirements:
  • Possess a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization
  • Licensed to practice as a physical therapist in the State of Wisconsin
  • Two years of appropriate experience as a physical therapist; community or home health experience is preferred
  • Licensed driver with an insured automobile in good working order
  • Current CPR certification
