Clinical Support Coordinator

We're seeking a Registered Nurse (RN) who shares our passion for delivering quality care and creating meaningful patient experiences. As a member of our team, you'll be responsible for evaluating patients and creating tailored care plans, ensuring effective communication among patients, their families, and the care team. This role requires strong clinical skills, excellent communication and interpersonal skills, and a commitment to creating positive patient experiences.
Responsibilities:
- Evaluate patients and create personalized care plans
- Ensure effective communication among patients, families, and the care team
- Drive quality care and create positive patient experiences
- Collaborate with the care team to ensure seamless care delivery
Requirements:
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
- Reliable transportation
- Ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs
