Key Responsibilities:
- Collaborate with healthcare teams to assess patient needs and develop personalized care plans.
- Coordinate patient care processes to achieve optimal quality outcomes and reduce resource utilization.
- Facilitate patient education and promote continuity of care to achieve optimal patient outcomes.
- Participate in multidisciplinary team care conferences to discuss complex patient cases and develop comprehensive care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and effective care transitions.
Requirements:
- Associate's Degree (or higher) in Nursing or related field.
- Valid unrestricted RN License in the state of Colorado or Master of Social Work (MSW) Degree.
- Ability to commit to working 1 weekend day per pay period (1 weekend day every 2 weeks).
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN) Degree.
- Case Management Certification.
- 3+ years of recent clinical or case management experience in area of specialization.
- Knowledge/understanding of community resources, policies, and procedures.
Soft Skills:
- Solid analytical, critical thinking, and organizational skills.
Colorado Residents Only: The hourly range for Colorado residents is $28.03 to $54.95 per hour. Pay is based on several factors, including local labor markets, education, work experience, certifications, and more. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
