Job Summary

The Clinical Pharmacist will be responsible for the daily operations of the pharmacy medication management systems, including ward stock, unit dose, and outpatient prescription dispensing. The ideal candidate will have a strong understanding of pharmacy practice and be able to communicate effectively with patients and healthcare staff.

Key Responsibilities
  • Review and interpret medication orders for appropriateness
  • Process and fill medication orders
  • Interact with and make recommendations to other clinical staff regarding medication therapy
  • Review patient medications, allergies, labs, and other pertinent information to identify and solve medication-related problems
  • Work with providers, nurses, and other healthcare professionals to ensure optimal patient medication needs
  • Screen for community care consults and approvals, and enter community care provider profiles and process orders associated with community care
  • Investigate and provide good patient medication issue solutions
  • Maintain automation equipment to ensure accurate filling and dispensing
  • Fills and/or verifies pharmacy technician non-sterile compounding orders
  • Processes, fills, and verifies scheduled and non-scheduled federal legend drugs and medical devices, over-the-counter products, and supply items
  • Contact providers as appropriate
  • Document recommendations and interventions
  • Provide refill extensions and partial medication supplies as appropriate
  • Take health and medication histories
  • Perform medication reconciliation
  • Provide drug information
  • Submit QA forms as needed locally or to CMOP
  • Assist in formulary management, including therapeutic substitutions, non-formulary reviews, and medication usage evaluations
  • Document and assess adverse drug events (ADEs)
  • Assist in medical emergencies
  • Provide emergency fills for patients as appropriate
  • Counsel patients on medications and related matters
  • Provide good customer service when communicating with healthcare workers and patients
  • Operate pharmacy software, providing oversight of technical staff in all aspects of medication distribution
Requirements
  • United States Citizenship
  • English Language Proficiency
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
  • Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
  • Ability to communicate orally and in writing to both patients and healthcare staff
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security
  • Ability to monitor and assess the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Work Environment

The Clinical Pharmacist will work in a fast-paced pharmacy environment, interacting with patients, healthcare staff, and other professionals. The ideal candidate will be able to work effectively in a team environment and provide excellent customer service.
