Job Title: Home Health Therapist

Job Description:

We are looking for a highly motivated Physical Therapist to work in an institutional setting, providing expert physical therapy treatment and training for patients with various injuries, illnesses, or functional disabilities. Key Responsibilities:

  • Plan and administer prescribed physical therapy treatment and training for patients.
  • Test and screen patients' physical strengths to assist physicians in evaluating their level of function and recording findings.
  • Develop and implement conditioning/rehabilitation programs consistent with physician plans of treatment.
  • Confer with physicians and clinical team members to obtain additional patient information and assist in developing therapy treatment programs.
  • Provide written instructions/care plans to Physical Therapy Assistants and Home Health Aides.
  • Accurately document patients' care observations, interventions, and evaluations.

Requirements:

  • Advanced degree of Doctor of Physical Therapy (DPT) from an institution accredited by the American Physical Therapy Association (APTA).
  • Current and unrestricted license to practice Physical Therapy in the State of California.
  • Current CPR/BLS certification.
  • Possess up-to-date knowledge of treatment practices and physical therapy programs.
  • Ability to assess patient condition and work with treatment plans.
  • A minimum of 1 year of recent Physical Therapy experience in an institutional setting.
