SUMMARY/OBJECTIVE

The Community Support Specialist plays a vital role in assisting individuals in achieving their personal goals and maintaining their health. This position involves providing support in daily living activities, facilitating community engagement, and empowering individuals to build meaningful relationships.

This role may include a combination of in-home support and group activities.

PREFERRED EDUCATION and EXPERIENCE

  • High School Diploma or GED
  • Valid driver's license and dependable transportation
  • At least 1 year of experience working with individuals with disabilities (preferred)
  • Strong interpersonal skills, demonstrating courtesy and good judgment
  • Clean driving record
  • Possession of appropriate safety equipment and first aid kit in personal vehicle

ESSENTIAL DUTIES and RESPONSIBILITIES

  • Implement cognitive interventions and train individuals in essential personal care skills (e.g., toileting, bathing, grooming, dressing, eating).
  • Maintain professional communication with individuals served, their families, and support systems while establishing clear boundaries.
  • Assist with personal care and activities of daily living (ADLs), ensuring hygiene and nutritional needs are met.
  • Provide training on utilizing community resources, including transportation, shopping, and recreational activities.
  • Monitor health conditions and assist with medication management as required.
  • Recognize and report any signs of abuse, neglect, or exploitation.
  • Supervise individuals to ensure their health and safety.
  • Facilitate behavioral support and adaptive skills training for home and community settings.
  • Encourage active participation of individuals and their families in the service delivery process, documenting progress towards daily goals.
  • Assist individuals in navigating developmental transitions and connecting them with community resources.
  • Complete necessary documentation promptly and maintain accurate timekeeping.
  • Ensure personal vehicle insurance and registration are current.
