Community Habilitation Field Coordinator

Role Summary: The Community Habilitation Field Coordinator plays a vital role in ensuring the effective management of individual support services. This position involves regular assessments and follow-ups to ensure the highest standards of care and community engagement.
Key Responsibilities:
- Conduct thorough evaluations to establish individual goals.
- Oversee Community Habilitation staff and address sensitive matters with professionalism.
- Maintain communication with families regarding the progress of individuals.
- Monitor daily attendance and engagement.
- Regularly update families and staff on program developments and maintain open lines of communication with the Department Director.
- Promptly inform the Department Director of any operational issues.
- Plan and schedule individual visits in advance, documenting them in the weekly planner.
- Collect annual feedback from families through surveys.
- Compile and analyze data for individual progress and billing documentation.
- Perform monthly visits to ensure staff adherence to established goals.
- Execute additional tasks as assigned by the Department Director.
- Participate in scheduled meetings as required.
- Train new and existing staff on the creation of goal documentation.
- Investigate incidents, both reportable and non-reportable.
- Complete monthly service documentation for individuals.
- Gather necessary documentation from relevant service coordinators.
- Assist with COVID documentation, Telehealth services, and Electronic Visit Verification (EVV) implementation.
Qualifications: A Bachelor's Degree in a Human Services-related field is preferred. Candidates must hold a valid New York State Driver's License. A minimum of one year of experience working with individuals with developmental disabilities is required. Leadership experience, including staff supervision and program management, along with proficiency in documenting individual progress, is essential. Strong skills in community inclusion and excellent verbal and written communication abilities are necessary.
