Pediatric Care Coordinator Role

La Rabida Children's Hospital is seeking a skilled Pediatric Care Coordinator to join their team. In this role, you will be responsible for working with patients and families to provide clinical support and manage ongoing health treatment plans. Key Responsibilities:
  • Evaluate patient needs and determine the appropriateness of services provided at La Rabida.
  • Partner with providers and charge nurses to optimize patient outcomes and health.
  • Provide intermittent patient follow-up between clinic visits.
Requirements:
  • Graduate degree in professional nursing (BSN required, MSN preferred).
  • Three years of pediatric nursing experience (care management or clinical specialty desirable).
  • Current Illinois Professional Nursing Licensure and certification in pediatric nursing, case management, or specialty preferred.
