We are seeking a highly skilled Physical Therapist to join our team at Heartland Home Health. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our interdisciplinary team.
Key Responsibilities- Develop and implement individualized physical therapy treatment plans to help patients achieve optimal physical function and independence.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care and coordinate treatment plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment strategies.
- Provide education and training to patients and their families on proper techniques and exercises to promote optimal recovery and wellness.
- Master's degree from an accredited Physical Therapy Program.
- Current licensure as a Physical Therapist in the state of practice.
- Current CPR certification.
- One year's experience in hospital or home health preferred.
