Position: Practical Nursing Specialist

Key Responsibilities:

  • Collaborate with team members to develop and implement clinical treatment plans under the supervision of healthcare providers and registered nurses.
  • Support clinical services in home and field settings, including co-visits and telehealth consultations.
  • Conduct health assessments for new and existing patients.
  • Perform clinical tasks such as wound care, phlebotomy, and medication reviews.
  • Engage with patients in their homes to ensure continuity of care.
  • Provide coverage for team members during planned and unplanned absences.
  • Work with interdisciplinary teams to enhance care quality and patient satisfaction.

Qualifications:

  • Licensed Practical Nurse with certification in the state of employment.
  • Experience in holistic care, including support for patients with mental health and substance use challenges.
  • Current CPR and BLS certification.
