Job Summary

We are seeking a highly organized and compassionate Care Coordination Specialist to join our team at Sweet Virginia Care LLC. As a key member of our staff, you will play a critical role in ensuring that our home care clients receive the highest level of service and attention.

In this rewarding role, you will serve as the main contact for clients, families, and caregivers, addressing questions, concerns, and feedback promptly. You will schedule and coordinate client care plans, match caregivers to clients' needs, and manage phone and email communication. About the Role
  • Serve as the main contact for clients, families, and caregivers, addressing questions, concerns, and feedback promptly.
  • Schedule and coordinate client care plans and match caregivers to clients' needs, ensuring client needs are met.
  • Manage phone and email communication, data entry into the scheduling system, and general office tasks.
  • Communicate with caregivers to ensure timely and effective service delivery.
  • Support the recruitment team in interviewing potential caregivers that meet the needs of clients.
Requirements
  • High School Diploma required (Bachelor's preferred);
  • 2 years experience as a care coordinator or in a similar role is preferred, but we are open to training someone who has limited experience if they are a good fit;
  • Strong organizational, planning, and communication skills;
  • Ability to work independently and as part of a team;
  • Compassionate, empathetic, and patient in interactions with clients and families;
  • Knowledge of HIPAA and healthcare office regulations;
  • Time management and prioritization are key;
  • Ability to multi-task and handle multiple phone lines;
  • Computer skills are a must, including outlook
