Job Summary

We are seeking a skilled Physical Therapist to join our team at Home Recovery-Home Aid. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Conduct thorough assessments to determine client functional level, including designated OASIS items for coordination of care among other disciplines.
  • Develop and implement personalized physical therapy plans to achieve client goals, working closely with the client's physician and Agency staff.
  • Evaluate equipment needs to enhance client functionality and provide recommendations for improvement.
  • Monitor and report client progress to the physician and Agency staff, ensuring seamless communication and coordination of care.
  • Collaborate with nurses, home health aides, caregivers, and clients to provide a comprehensive physical therapy program.
  • Participate in clinical conferences and in-service programs to share best practices and stay up-to-date on industry developments.
  • Deliver services in accordance with accepted standards of treatment, tailored to each client's unique needs and conditions.
  • Assist in the development and revision of the plan of care, working closely with the Agency staff and physician.
  • Consult with the Nurse Case Manager, Director of Professional Services, and VP of Clinical Services to ensure smooth discharge planning and notification.
  • Maintain accurate and comprehensive clinical records, documenting client limitations and progress.
  • Develop and instruct Agency staff and caregivers in maintenance therapy programs, as approved by the physician.
Requirements
  • Current Physical Therapy License in VA.
  • Graduation from an approved physical therapy program.
  • One-year clinical experience.
  • Current CPR certification.
  • Excellent communication, organizational, and human relations skills.
  • Ability to function with minimal supervision.
  • Capacity to manage multiple projects and changing priorities.
  • Employment of critical thinking skills.
