We are seeking a highly skilled Registered Nurse Specialist to join our team at the Levy County Health Department. As a key member of our Healthy Start program, you will provide comprehensive nursing care to pre and post-natal women, as well as offer case management for infants and their families.
Responsibilities- Provide professional nursing care to pre and post-natal women, including case management for infants and their families.
- Lead the Healthy Start program team and work under the direct supervision of the Registered Nurse Supervisor.
- Coordinate and facilitate Healthy Start care coordination that meets minimum standard requirements and assures risk-appropriate care under the new Medicaid Waiver Program.
- Complete PEPW applications for eligible clients and review Healthy Start screening instruments, statistical data, and reports.
- Teach educational classes, such as Childbirth Education, Parenting, Breastfeeding, and Smoking Cessation Support groups and classes, in response to client requests and needs.
- Perform home visits to Healthy Start clients and provide individualized education and developmental screenings.
- Assess individual clients' needs and develop Individualized Plans of Care and follow-up care.
- Refer clients to appropriate resources when indicated and implement QI review per Healthy Start guidelines.
- Clear and Active Florida Registered Nurse License.
- Valid driver's license and reliable transportation.
- Basic computer skills and good written and verbal communication skills.
- Knowledge of community resources and ability to effectively interact with the community.
- Experience working in HMS in a County Health Department.
- Experience working with pregnant women and their families.
- Experience working with children from birth to 3 years old and their families.
- Experience working in a position that requires client home visits and case management experience.
