Job Summary

We are looking for a dedicated Physical Therapist to provide personalized rehabilitation therapy to patients in their homes or care facilities. The ideal candidate will have a minimum of one year of physical therapy experience or have successfully completed a probationary period as a VNS Health Rehab Intern.

Responsibilities
  • Deliver high-quality rehabilitation therapy to patients in their homes or care facilities.
  • Work independently as part of an interdisciplinary care team.
Requirements
  • Licensure to practice as a Physical Therapist in New York State.
  • Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
