In this critical role, you will be responsible for reviewing and analyzing information for medical necessity criteria, serving as a liaison between sending hospitals and Avera's Transfer Center RN and accepting physician. Your goal will be to ensure timely and efficient patient transfers, while maintaining high-quality care standards.
Main Responsibilities:
- Verify the appropriateness and necessity of admission to the acute hospital for all patients.
- Coordinate and communicate with inter-disciplinary teams to identify, plan for, and provide follow-up care for all patients.
- Monitor documentation of patient care to assure evidence-based plans of care.
Requirements:
- BSN or related field.
- Active license in the state of practice.
- 4-6 years related experience and/or training.
