* Connecting patients and families with community resources
* Collecting and reporting financial indicators
* Participating in clinical performance improvement activities
Requirements
A Master's degree in Social Work from a CSWE-accredited program and licensure in Illinois as a Licensed Social Worker (LSW) are required. Excellent interpersonal and organizational skills, strong analytical and problem-solving abilities, and effective communication skills are also essential.
