Position Highlights:

  • Exemplifies compassion, awareness, and exceptional communication skills to support primary care teams in an outpatient context.
  • Partners with leadership and operational teams to design, advocate for, and implement clinical policies that promote integrated, patient-centered primary care.
  • Conducts RN support tasks, including but not limited to:
    • Reviewing urgent matters within the Care Team's desktop, addressing requests, lab results, and more.
    • Managing patient phone communications, providing instructions, and addressing inquiries as directed by the primary care provider.
    • Processing prescription refills in accordance with established standing orders.
    • Contributing to quality improvement initiatives by analyzing quality reports and collaborating with Quality Business Partners on identified improvement areas.
    • Maintaining precise, timely, and comprehensive documentation of all patient interactions, whether in-person, remote, or via telephone.
  • Collaborates effectively across teams, fostering trusting relationships while ensuring compliance with professional standards.
  • Demonstrates flexibility in meeting departmental needs, providing coverage for other Care Teams as necessary.
  • Works within teams to ensure that Primary Care Team RN services are centered on patient needs and aligned with our mission, advocating for continuous improvement and best practices.

Qualifications:

  • Must possess an unrestricted Registered Nurse license to practice in the State of Maine or the ability to obtain licensure within one year of hire.
  • Preferred: Minimum of three years of nursing experience in inpatient, home health, hospice, or primary care settings.
  • Experience in a large, complex, multi-stakeholder environment is advantageous.
  • A valid driver's license with a satisfactory driving record is required.
