Job Description: As a Home Health Occupational Therapist, you will assess and develop therapeutic retraining programs for patients in their homes, collaborating with physicians to create and adjust treatment plans. You will provide direct patient care to improve daily living skills and participate in community reintegration activities. Additionally, you'll coordinate with interdisciplinary teams and supervise Occupational Therapy Assistants while ensuring accurate documentation of patient care. This role requires a high level of clinical expertise, strong communication skills, and the ability to work collaboratively as part of a team.

Key Responsibilities:
  • Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function.
  • Confer with the patient's physician and other healthcare team members and participate in development/revision of the Plan of Care Treatment.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.

Required Skills and Qualifications: To be successful in this role, you will need a degree from an accredited Occupational Therapy program, a minimum of six months of occupational therapy experience, current and unrestricted OT licensure, current CPR certification, good organizational and communication skills, and a valid driver's license, auto insurance, and reliable transportation.
