Job Title: Clinical Social Worker - Discharge and Community Integration Specialist

This position is located at Coalinga State Hospital in Fresno County and reports to the hospital Chief of Social Work Service.

Job Summary:

The Discharge and Community Integration Specialist is a coordinator of hospital-based discharge efforts for civil and forensic patients within the Department of State Hospitals (DSH). The specialist will work closely with the statewide DSH Program Director for Discharge and Community Integration (DCI) to develop and implement strategies for discharge and community integration.

Key Responsibilities:
  • Develop and implement discharge plans for civil and forensic patients
  • Collaborate with the Forensic Conditional Release Program (CONREP) to ensure seamless discharge and community integration
  • Provide training and resources to hospital staff on discharge and community integration best practices
  • Monitor and evaluate the effectiveness of discharge and community integration efforts
  • Develop and maintain relationships with community-based organizations and providers to ensure a smooth transition for patients
Requirements:
  • Be LIST ELIGIBLE: Successfully complete the competitive State civil service exam pertaining to this classification
  • Be TRANSFER ELIGIBLE: Have current employment with the State of California in the same classification or in a classification that is comparable
  • Be REINSTATEMENT ELIGIBLE: Have obtained previous permanent employment with the State of California in the same classification or in a classification that is comparable
Working Conditions:
  • Occasional, limited travel may occur and reimbursement will be in accordance with BU agreements and standard travel policies
  • Hybrid telework is available based on operational needs and is subject to change, with a current minimum three in-office work days per week requirement
  • The work hours for this position will be set sometime between 8:00 am and 5:00 pm Monday through Friday
