**Job Summary:**
Coordinate patient care and services to ensure seamless delivery of high-quality care in the home setting. Develop and implement individualized care plans, conduct thorough assessments, and collaborate with healthcare teams to ensure effective care coordination.

**Responsibilities:**
• Develop and implement individualized care plans for patients
• Conduct thorough assessments and identify patient needs
• Collaborate with healthcare teams to ensure seamless care delivery
• Communicate effectively with patients, families, and healthcare teams

**Requirements:**
• Current Oregon Registered Nurse License upon hire
• National Provider BLS - American Heart Association within 30 days of hire
• Must be licensed in states where care is delivered upon hire

**Compensation and Benefits:**
This full-time position offers a competitive salary range of $39.78 - $62.96 per hour, depending on experience.
