Job Description

We are actively seeking a Community Integration Counselor in Butler, PA to join our team. As a Community Integration Counselor, you will work closely with individuals to assist them in managing and overcoming the difficulties of living in the community. Your primary responsibility will be to provide individualized services designed to promote independence and community integration.



Key Responsibilities

  • Support and encourage individuals to increase their ability to be as independent as possible
  • Meet with waiver participants on a regular basis to discuss their needs and develop personalized plans
  • Collaborate with case managers to develop and facilitate programs that meet the social, emotional, physical, and personal needs of individuals
  • Provide ongoing support for community integration efforts and document individual progress
  • Assist in building a support network between the individual and the community
  • Ensure medical consultation forms are completed and medications are administered properly
  • Coordinate leisure and volunteer opportunities for individuals
  • Monitor the health and medical needs of individuals and report any concerns
  • Drive individuals to medical appointments and activities in the community
  • Ensure program documentation and billable records are completed accurately and timely
  • Additional tasks and responsibilities may be assigned


Minimum Requirements

  • High School Diploma or GED
  • Bachelor's Degree preferred
  • Valid driver's license with a satisfactory driving record
  • Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to exercise good judgment and remain calm in crisis situations
  • Excellent verbal and written communication skills
  • Excellent time management skills


Working Conditions

  • Transport individuals using your own vehicle or company provided vehicles
  • Schedule may change due to business needs and may include evening and weekend hours
  • May be required to be on-call in cooperation with other management staff
  • Frequent lifting, stretching, and other physical exertion may be required
  • May be required to lift or move 25+ pounds
  • May be exposed to various medical conditions and communicable diseases

Estimated Salary

The estimated salary for this position is $16.00 per hour, with opportunities for growth and professional development. New hires are eligible for a sign-on bonus of $250.00 payable after working all scheduled hours with no unexcused call outs, complete all required trainings, and fulfill all required tasks. Additionally, must successfully pass the 90-day introductory/probationary period per the ED's approval including performance standards (maintain 16+ hours a week).
