Join our team as a Homecare Social Worker III-D and make a difference in the lives of our patients.

About the Position:

We are seeking a highly skilled Homecare Social Worker III-D to join our team at Covenant Home Care. In this role, you will provide professional social work services to designated patient populations, including counseling and discharge planning.

Key Responsibilities:

  • Assist patients/family/significant other with physical, emotional, and social concerns that arise during the episode of care.
  • Provide discharge planning services to patients needing this service.
  • Complete required documentation prerequisites for placement/authorizations prior to discharge.
  • Complete documents that require a licensed individual to complete.
  • Provide oversite in a situation where a licensed individual is required.
  • Provide expert training in the field to others, as requested.
  • Complete initial assessment and continued documentation to obtain pertinent background/social/financial data.
  • Connect patients/family/significant other with community resources to aid in their treatment and recovery.
  • Act as facility liaison with adult and children protective services as indicated.
  • Collaborate with interdisciplinary team including physicians and keep them informed of progress with patients via verbal communication and chart documentation in medical record.

Requirements:

To be successful in this role, you must have a Master's degree from an accredited graduate social work program and two years of experience in a related field. You must also possess one of the following licenses: LMSW, LAPSW, or LCSW. A valid Tennessee driver's license and state mandated minimum insurance coverage are also required.
