Cascade Community Healthcare (CCH) is dedicated to fostering a community built on acceptance, compassion, and respect. With over five decades of experience in providing essential treatment services in Lewis County, we are in search of a Practical Nursing Specialist to enhance our Outpatient Medical Team. The successful candidate will play a crucial role in delivering medical assistance to enrolled clients, focusing on medication oversight, treatment coordination, and collaboration with primary clinicians and case managers.
- Possession of a valid LPN credential from the Washington State Department of Health is required.
- A minimum of two years' experience in supporting individuals with severe and persistent mental health conditions is preferred.
- Experience in collaborative work with other service providers is advantageous.
- Strong professional representation of the agency and effective communication skills are essential.
Key Responsibilities:
- Provides comprehensive support to clients, the medical department, and the Director of Nursing.
- Facilitates treatment services in coordination with relevant agencies and the client's support network.
- Ensures accurate record-keeping in alignment with agency policies.
- Engages in supervision, consultation, and training as directed.
