About the Role:

We are seeking a skilled Healthcare Specialist to join our team at Memorial Hermann Health System. This role is responsible for providing comprehensive patient care through assessment, diagnosis, treatment, and management.

Key Responsibilities:

  • Conduct thorough assessments of patients' needs, developing and implementing effective care plans
  • Collaborate with interdisciplinary teams to ensure seamless patient care
  • Develop and manage treatment plans, prioritizing patient outcomes
  • Maintain accurate records and reports, contributing to quality improvement initiatives
  • Stay updated on industry developments, applying new knowledge to enhance patient care

Requirements:

  • Graduate of an accredited Nurse Practitioner Program
  • Certification by a recognized credentialing organization (e.g., ANCC or NCC)
  • Current licensure to practice professional nursing in Texas
  • A minimum of 2 years clinical experience in palliative medicine

Compensation: $132,500 - $165,000 per year, commensurate with experience
