Advanced Practice Provider for Primary Care Weekends

Job Summary:

RWJBarnabas Health is seeking an Advanced Practice Provider to join our team in a full-time position providing primary care on weekends. This role will involve working independently to assess, diagnose, and manage patient health status, as well as collaborating with multidisciplinary teams to develop and implement care plans.

Responsibilities:

  • Perform duties independently following established procedures and methods, referring questionable situations to physicians.
  • Provide primary health care through assessment, diagnosis, clinical judgment, and management to restore, maintain, and improve patient health status.
  • Develop and implement care plans in coordination with multidisciplinary teams, completing reports to physicians and family as necessary.
  • Document and maintain patient records of services provided, adhering to facility and nursing standards.
  • Establish a collaborative relationship with other medical providers, examining patients to determine the presence of illness and prescribing treatments as indicated.
  • Interview, instruct, and advise patients regarding health and illness prevention, reviewing lab data and documenting findings.
  • Perform and obtain completed medical histories and physical data on patients, with appropriate documentation and findings.
  • Provide monitoring and continuity of care between physician visits, responsible for coordinating nursing and training programs on compliant policies such as HIPAA, OSHA, and Code of Conduct.
  • Maintain the quality and adequacy of nursing practice, responsible for reporting identified safety issues such as hazardous environments and medical errors.

Requirements:

  • Must have previous ICU experience, including central lines, arterial lines, chest tubes, and critical care management of surgical/trauma patients.
  • Ability to function independently and work as part of the ICU surgical team.
  • 3 years of surgical/trauma critical care experience.
  • Variable shift, including days, nights, evenings, holidays, and weekend rotations.
